Empowers adults living with mental illness and/or developmental disabilities, offers various programs aimed at fostering healthy choices, stabilizing symptoms, and realizing full potential. Services encompass mental health case management, supportive housing, homeless services, jail diversion, senior mental health and wellness, intensive outpatient and partial care services, developmental disabilities services including adult day programs and group homes, community supportive living services, and transitional housing for homeless veterans at Veteran's Haven.

Provides a day program that serves individuals with various disabilities, with a focus of medical supports, without nursing support. The program is 1:3 staff ratio. There are various station areas, a changing room and a "quiet area" for individuals who may need a less stimulating environment. . The station areas include a nutrition station; Health and Wellness, Life skills station; and Technology.
Enables participants to live at home in the evenings and on the weekends while receiving individualized care for their medical, nursing, social, rehabilitative and personal needs at the centers. Provides adults ages 21 and older with developmental disabilities who need a daily program of activities to develop skills in adaptive living, self-help, and personal awareness. The program offers on-site and off-site community-based activities to clients and assists them with finding employment opportunities and/or volunteer positions for those who express a desire to either work or volunteer in the community.
Offers adult medical daycare services to the elderly, adults with developmental disabilities, and adults with dementia. Complimentary transportation to and from the facility and medical appointments is provided at no extra charge. Meals are prepared on-site and supervised by a registered dietician.

Delivering services at a 1:3 ratio, the Sparta Center supports individuals with substantial physical or intellectual support needs, including those considered medically fragile. Nursing staff monitors conditions like diabetes and cardiac or respiratory issues. The center offers person-centered daily routines emphasizing social interaction, leisure activities, and learning, featuring programs on music, nature, and art. Thoughtfully arranged community trips recognize individual interests and participants.
Provides a senior recreation and leisure program for individuals who wish to be retired. The program follows a daily schedule which consists of current events, exercise, reading groups, arts and crafts along with community outings. The program has many groups led by the individuals, but not limited to: Reading group, recycling club, choir, and the supply team. The Senior Program provides 4 lessons monthly safety, hygiene, health and nutrition, and a cultural presentation quarterly.
Provides social day care services for older adults or those with disabilities. Services can include counseling, exercise programs, field trips, hot meals, information and referral, nursing services, pharmaceutical consultation, therapeutic recreation, physical, occupational and speech therapy, and support groups.
Focuses on building and improving daily living skills for adults with developmental and/or multiple disabilities. Programs and activities available at the center include: Case management, Comprehensive adult education, Pre-vocational exploration, Nursing services, Assistive technology evaluation and training, Hearing and vision screenings, Dental clinic, and Recreation and community inclusion programs.
