Case management services is available.Participants in the program can also receive workshops and home assessments.

Available via pager 24/7 for emergency consultation. Provide professional and community training and education regarding the impact of violence on children and intervention strategies.

Offers services such as planning and implementing escapes (including internationally), providing free legal representation for divorce, child-custody, immigration, name-changes, psychotherapy, transportation, and financial and career counseling.

This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues.

The Jewish Family Service of Greater Clifton-Passaic offers case management and counseling services for older adults in the community. Social workers assist senior citizens with matters such as finances, health care, and government benefits. In addition, seniors can receive weekly counseling for matters relating to life challenges, loss/grief, and family issues.

Provides HIV clinical trials and diverse psychosocial services to HIV-positive individuals, their families, and friends, including counseling in mental health, nutrition, substance abuse, and treatment education. Additionally, services entail HIV-related support groups for men, women, gay/lesbian individuals, and the Double Trouble group for those with HIV and addictions, along with case management, prescription assistance, social therapy, van transportation, and outreach to HIV-positive individuals and those at risk, which includes HIV and STD testing and counseling.

Provide HIV testing, primary medical care, case management, and prescription drug assistance to HIV-positive individuals from Union County at Trinitas Regional Medical Center. Deliver comprehensive medical care by infectious disease specialists, gynecology referrals, medication management, and coordination of inpatient and outpatient care. Administer pneumonia, flu, and hepatitis vaccines, screen for Tuberculosis, and offer on-site care Monday through Friday. Provide education on medications, treatments, and risk reduction. Case management includes counseling, referrals to substance abuse and psychiatric services, financial and benefit counseling, food and housing assistance, prescription aid, and support for permanency planning of minor children with ongoing follow-up.

Offers a long-term approach to recovery requiring daily participation for medication with weekly counseling. Treatment services offered include:- Individual and group counseling- Family counseling (family members are invited to participate in treatment when appropriate)- HIV/HEP C counseling, testing, referrals, and case management services- HEP A & B Vaccines (available upon request)- Case management and referral services as needed

Helps individuals, ages 18 and over, diagnosed with a severe mental health condition successfully integrate into the community, with a focus on achieving and sustaining independence for personal fulfilment. Services include: advocacy, linkage and referral, relationship building, daily living skills, and development of community-based supports.

Assists adults with serious and persistent mental illness and/or co-occurring substance use disorders increase independence and promote community integration. Services provide linkages to help secure necessary services and resources while maintaining recovery and quality of life. Participants can receive services for up to 12 months.

Families In Motion (FIM) is a multi-agency collaborative focused on supporting families in Cumberland and Gloucester Counties on their journey to success by providing them with the tools and resources needed to achieve their economic, health, and other family-related goals. Available services include case management and English as a Second Language (ESL).

Provides a variety of free services to individuals with HIV/AIDS, including HIV case management, individual and group counseling, a weekly dinner/support group, and opioid abuse treatment. Those enrolled in the case management program may access emergency financial assistance for needs such as utility shut-off, lack of food, or lack of medication.

Services include, but are not limited to: skilled nursing care; prescription assistance; physical, occupational, and speech therapy; home health care; case management; respite care; social and medical adult day care; chore services; transportation to physicians' offices and other medical facilities; and medical social work services, including home assessments.

Offers home care programs that include in-home skilled nursing, certified home health aides, physical, occupational, and speech therapy, medical social workers, nutritionists, infusion, bereavement support groups, and a children's bereavement camp.
