Case Management - Hudson County Office | Heightened Independence and Progress (HIP)

Provides assessments, direct services, and referrals to community based programs and coordination of services. Services include: assistance in applying for SSDI, SSI, Medicare, Medicaid, Managed Long Term Support Services (MLTSS); assistance with applications for food stamps, PAAD, LIHEAP, Meals on Wheels, affordable housing, durable medical equipment, and home modifications. Also provides individual and family support and linkages to home care, respite care, and caregiver support.
