Facilitates community support, advocates for services, and monitors service provision to enhance independent living for individuals diagnosed with a mental illness. In the transition from psychiatric hospital to successful community living, ICMS provides support, advocacy, and linkage to services. ICMS services encompass:- Providing supportive counseling systems advocacy- Delivering wellness and recovery education- Conducting individual assessment- Developing and implementing recovery plans- Linking individuals to needed services- Facilitating continuity of care- Coordinating services with other agencies ICMS focuses on enhancing the physical, emotional, and intellectual strengths of individuals served, identifying and utilizing community resources to help consumers achieve their goals. Recovery entails changing attitudes, values, feelings, skills, goals, and/or roles to live a satisfying, hopeful, and contributing life despite limitations caused by illness.
