Eligibility

Open to individuals who have been diagnosed with a mental illness. Applicants can qualify if they: - Have been admitted to the state or county hospital system - Are ICMS recipients who move from one service area to another - Have had two or more admissions to a Short Term Care Facility - Have had repeated admissions to local psychiatric hospitals within a 12 month period - Have had two or more uses of Psychiatric Emergency Screening Services within a 30 day period
