Eligibility

Open to adults, ages 18 or older, with a history of psychiatric hospital admissions for serious and persistent mental illness and impaired functioning who have been approved by the Division of Mental Health Services (DMHS). Eligible diagnoses include:- Schizophrenia/other Psychotic Disorders;- Schizoaffective Disorder;- Major Depressive Disorders;- Bipolar Disorders;- Delusional Disorders

Aims to decrease hospitalizations, enhance vocational opportunities, and improve overall quality of life, offering lifelong support when necessary.
