Provides assistance for medical needs to Union County residents. Assistance includes NJ Family Care and Medicaid enrollment, as well as special programs for Medicaid-qualified individuals such as Long Term Care expense assistance, hospice programs, waivers for AIDS and Brain Injury patients, and non-emergency medical transportation.

Provides medical coverage to elderly and disabled individuals in need of nursing home/assisted living care or already residing in one of these institutions. Programs include Medicaid Nursing Home Program, New Jersey Care Nursing Home Program, and Medically Needy Nursing Home Program.

Provides in-home support to brain-injured individuals, who are currently in nursing facilities or who are in the community and at risk for placement in nursing facilities. Reintegrates into and maintains individuals in the community so that supervision is no longer needed, or is minimized. Primarily serves ambulatory young adults with cognitive, behavioral, and physical deficits which require supervised and supportive care.
