Description
Provides in-home support to brain-injured individuals, who are currently in nursing facilities or who are in the community and at risk for placement in nursing facilities. Reintegrates into and maintains individuals in the community so that supervision is no longer needed, or is minimized. Primarily serves ambulatory young adults with cognitive, behavioral, and physical deficits which require supervised and supportive care.

Also provides comprehensive health coverage and a wide-range of services including: doctor visits, hospital services, prescriptions, tests, vision care, mental health care, dental, nursing home care and other healthcare services, depending on the person's eligibility category.
