Enables individuals to stay or return to their community instead of residing in a long-term care facility through a federal home and community-based program. It provides limited covered services such as home health, adult day care, transportation, case management, homemaker assistance, prescription aid, and respite care. Additionally, it includes the Caregiver Assistance Program (CAP) and the Assisted Living/Alternate Family Care program for those needing extra support.
