Offers a team-based program for individuals with serious and persistent mental illness and substance use disorders who have a history of multiple emergency room visits or psychiatric hospital admissions in the last 18 months. The PACT Teams provide intensive, in-community treatment and support to assist consumers in staying out of the hospital and improving their functioning in the community.
Offers free, intensive, short-term support for individuals facing a mental health crisis, presenting an alternative to inpatient hospitalization. Staffed 24/7 by trained professionals, the house transforms crises into opportunities for enhancing personal wellness. Aimed at inspiring hope, the service assists guests in developing crisis self-management skills for handling current and future situations. Emphasizing self-care in sleep, rest, relaxation, nutrition, physical activity, and co-occurring medical or substance use issues, the program allows guests to stay for a maximum of 10 days. Alternatively, in case of a waiting list or if a guest prefers staying at home, the service provides continued contact and outreach for up to one month.
