Benefits and Assistance Program | HOPES Community Action Partnership

Educates individuals on benefits and entitlement programs they may be eligible for and provide enrollment assistance through HOPES Community Action Partnership (CAP). Support is available for Social Security, Medicaid/Medicare, Food Stamps/SNAP, health education, and discount phone and utility services, including Verizon Lifeline service, Assurance/Safelink Wireless, and Home Energy Assistance Programs.

Advocacy - Bergen County Office | Heightened Independence and Progress (HIP)

Advocacy promotes and ensures the full inclusion of people with disabilities in the community. The staff directs efforts toward working with individual consumers, who face difficulties in areas of housing, employment, transportation, remaining independent in the homes, assists with completing applications for immigration, Social Security, and other financial benefit programs. Special services are also offered for underserved minorities who have disabilities.

Early Intervention Program (EIP) for HIV / AIDS | Trinitas Regional Medical Center

Provide HIV testing, primary medical care, case management, and prescription drug assistance to HIV-positive individuals from Union County at Trinitas Regional Medical Center. Deliver comprehensive medical care by infectious disease specialists, gynecology referrals, medication management, and coordination of inpatient and outpatient care. Administer pneumonia, flu, and hepatitis vaccines, screen for Tuberculosis, and offer on-site care Monday through Friday. Provide education on medications, treatments, and risk reduction. Case management includes counseling, referrals to substance abuse and psychiatric services, financial and benefit counseling, food and housing assistance, prescription aid, and support for permanency planning of minor children with ongoing follow-up.

Provides a variety of supportive programs to students of Perth Amboy High School and their families through the School Based Youth Services Program (SBYSP). Delivers intensive mental health counseling, including individual, group, and family sessions covering topics such as school, drugs/alcohol, abuse, family conflicts, peer pressure, relationships, eating disorders, and violence prevention.
