Case managers are available to provide a full array of social work services including individual and family counseling, support groups, referral to community services, client advocacy and pre and post HIV testing counseling.

Offers a wide range of general health care services. Services can include general health care, immunizations, lab testing, etc.

Provide HIV testing, primary medical care, case management, and prescription drug assistance to HIV-positive individuals from Union County at Trinitas Regional Medical Center. Deliver comprehensive medical care by infectious disease specialists, gynecology referrals, medication management, and coordination of inpatient and outpatient care. Administer pneumonia, flu, and hepatitis vaccines, screen for Tuberculosis, and offer on-site care Monday through Friday. Provide education on medications, treatments, and risk reduction. Case management includes counseling, referrals to substance abuse and psychiatric services, financial and benefit counseling, food and housing assistance, prescription aid, and support for permanency planning of minor children with ongoing follow-up.

Delivers comprehensive health services to individuals living with HIV/AIDS. This includes specialized medical care, case management, counseling, support groups, substance abuse therapy, medication management, health education, HIV counseling and testing by appointment, bilingual staff, advocacy, and absolute confidentiality.

Offers a wide range of general health care services. Services can include general health care, immunizations, lab testing, counseling, prenatal care, etc.

Provides a services for HIV patients including free HIV counseling, screening and testing services, primary medical care, dental care, mental health, and case management.

Offers comprehensive services for individuals affected by or living with HIV. This includes medical care, case management, counseling, and, when available, transportation assistance.

Offers comprehensive HIV primary medical and dental care for HIV positive clients, offering free and confidential rapid HIV testing, counseling, pediatric HIV/AIDS services, specialty medical care, prevention education, community outreach, medical case management, adherence counseling, psychosocial support, and nutrition services. On-site services also include social services, financial eligibility assistance, support groups, and community activities, with outpatient care provided by two physicians certified in infectious diseases and internal medicine who personally follow registered patients, including hospitalization if required.

Offers comprehensive services for individuals affected by or living with HIV. This includes medical care, case management, counseling, and, when available, transportation assistance.

Provides free, confidential HIV and STI testing, outreach, and health education. Also helps to identify and access medical care and supportive services, including treatment adherence, HIV prevention counseling, substance abuse counseling, and risk reduction counseling.

Free testing for HIV, Hep C, and Syphilis.

Offers a wide range of general health care services. Services can include general health care, immunizations, lab testing, counseling, prenatal care, etc.

Provides services for HIV positive and HIV negative individuals. Services include HIV prevention for men and women, treatment education, adherence support and medical care access, legal services, wellness program, connections to counseling and testing.

Offers medical services to the community. Services include, but are not limited to: adult medicine, pediatric care, podiatry, and AIDS/HIV care.

AIDS Outreach Program offers a medical care clinic and coordinators who provide support, counseling, and education for those infected and affected by HIV. Social services such as case management assistance with entitlements are also available.
