Eligibility

Older adults age 60 or older who:- require a nursing facility level of care but wish to remain at home, AND- are not financially eligible for Medicaid or Medicaid Waiver Services, AND- have resources at or below $40,000 for an individual, or $60,000 for a couple, AND- reside in a home or an apartment that is not a licensed residence.

Description

Provides in-home services for seniors at risk of placement in a nursing home. JACC is designed to supplement the assistance and individual receives from his/her natural support network. Qualified individuals may have the opportunity to hire their own eligible family, friends, or neighbors to provide the care they need.
