Eligibility

Eligible when person is united states citizen or qualified alien, is a resident of New Jersey, is 60 years of age or older, resides within the community (not in a facility), has been determined to require nursing facility level of care. Meets financial eligibility is having a monthly income that is not greater than 365% of the Federal poverty level ($4,581 for an individual; $6,217 for a married couple in 2024). Having countable assets total $40,000 or less for an individual or $60,000 or less for a couple.

Description

Offers in-home services to eligible individuals at risk of nursing facility placement. Services, determined by clinical assessment, encompass respite care, homemaker assistance, home modifications, emergency response systems, meal delivery, adult day care, medical equipment, transportation, and chore support.

Administers federal and State-funded services and supports and makes it easier for seniors and caregivers to live in the community as long as possible. The program assists with giving them independence, dignity and choice.
