Call to schedule an appointment for an initial assessment. A physician's referral will be required before attending the assessment. After the initial consultation, the patient will be moved into the group class portion of the program.
Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. Self-pay clients are charged a discounted rate of $19 per 30 minutes.Charity Care may be available through the Patient Financial Services department.

Description
A collaborative course through which people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. The course incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. DSME can be conducted on an individual and/or group basis, and is facilitated by a licensed health professional.
