Open to families reside in service area. Eligibility is based on clinical need and that decision is made by PerformCare. Typically, the CMOs throughout the state service youth, ages 5 through 21.

Application process

CCPFC is a contracted agent of New Jersey's Children System of Care (CSOC). As a result, all referrals to the Camden CMO / CCPFC must go through the contracted system administrator / PerformCare. To make a referral to CCPFC services, please call PerformCare for intake.
