Facilitating the transition from hospital to community living, Bridgeway's Residential Intensive Support Team (RIST) offers comprehensive, flexible, and mobile services. Supporting individuals' utilization of community mental health treatment, employment, and rehabilitation services, RIST provides:- Assistance in securing permanent housing with state-subsidized rental assistance vouchers- Needs assessment for developing wellness and recovery goals- Personal assistance emphasizing education and living skills development- Intensive outreach to engage recipients in mental health, substance abuse, and medical services- Ongoing coordination with other service providers- 24/7 staff availability and on-call rapid response- Support in developing peer and community social connections- Partnership with family/significant others- Career counseling, job-seeking skills training, and ongoing support- Clinically enhanced residential support staff, including a consulting psychiatrist, wellness nurse, housing specialist, and MICA and vocational counselorsRIST Goals:- Facilitate independent living within the community- Prevent unnecessary hospitalizations- Enhance quality of life, achieving goals like paid work, continuing education, supportive friendships, improved family relations, living alcohol and drug-free, and increased recreational opportunities.

Eligibility

The Middlesex RIST program accepts residents from Middlesex County who are homeless and currently on CEPP status at Trenton Psychiatric Hospital. Applicants must indicate a presence of a major DSM Axis I psychiatric illness and show a need for intensive support in order to maintain independent living.
