Description

Initiating as an extension of psychiatric rehabilitation programming, the Bridge to Wellness Integrated Care program furnishes individuals with psychiatric disorders comprehensive access to primary care health services, encompassing physical exams, diagnosis, treatment of medical conditions (e.g., diabetes, hypertension, obesity), vaccinations, on-site laboratory services, wellness programs, health education, pap smears, and referrals to specialists such as cardiologists, neurologists, and gynecologists. Its overarching objective is to dismantle barriers hindering access to vital primary care, thereby curtailing hospitalizations and preserving lives.
