Holy Name Medical Center | Diabetes Self-Management Education Classes (DSME)
Empowering individuals with diabetes, Diabetes Self-Management Education Classes (DSME) deliver collaborative training to enhance knowledge and skills for successful self-management. Guided by evidence-based standards and tailored to individual needs, goals, and life experiences, DSME is conducted by licensed health professionals. The program, available in both individual and group formats, covers diverse topics, including the disease process, treatment options, nutrition management, physical activity, medication use, blood glucose monitoring, complication prevention and treatment, goal setting, problem solving, psychosocial adjustment, and, if applicable, preconception care and pregnancy management. Individual sessions span 2 hours, while group classes occur over 2 days, each session lasting 3 hours.

Call to schedule an appointment for an initial assessment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one. Please call for more information.
Fee
Service fees of $102-162 per 30 mins apply. Most medical insurances, including Medicare and Medicaid, are accepted. Amount of coverage varies by insurance plan. Financial assistance and/or sliding scale fees based on income may be available for qualified participants. Please call for more information.
