Application process

Call to schedule an appointment. A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one.

Fees

Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. There is a prorated rate option available for uninsured clients. For Charity Care, call the hospital for more information on how to apply.

Description

Diabetes Self-Management Education (DSME) classes help individuals with diabetes learn how to manage their condition effectively through evidence-based practices. These classes, led by licensed health professionals, can be taken individually or in groups and cover topics such as nutrition, physical activity, medication use, and monitoring blood glucose levels. Additionally, DSME addresses goal setting, problem solving, and psychosocial adjustment to support overall health and well-being.
