Description
Assists individuals in their eligibility process for Supportive Living, helping them acquire skills for independent living such as home maintenance, personal hygiene, meal preparation, and budgeting. These services are offered as part of Community Based Supports or In Home Supports.

Provide assistance for individuals who need direct support to achieve independence and inclusion. Community Based Supports/Individual Supports are delivered in a one to one setting and may include: assistance with community-based activities, basic self-care, social skills and activities of daily living.
