Monmouth Medical Center - Lakewood | Diabetes Self-Management Education Classes (DSME)

Empowers individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training, guiding them to modify their behavior and successfully self-manage the disease. Facilitated by licensed health professionals, the course covers evidence-based topics such as the disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, prevention and treatment of acute and chronic complications, goal setting, problem solving, psychosocial adjustment, and preconception care and management during pregnancy if applicable. Classes can be conducted individually or in groups.

Application process

Please call to schedule an appointment. A physician's prescription with a diabetes diagnosis code will be required before registering. Those without a prescription can receive assistance in obtaining one. Please call for more information.

Fee

Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. For Charity Care, please call the hospital for more information on how to apply.

Eligibility

Individuals with gestational, type 1, and type 2 diabetes, and their families. Particular focus is on those who are newly diagnosed.

**Part of the Diabetes Initiative managed by the NJ Department of Health** DSME is for individuals who have been living with a Diabetes diagnosis for some time already, and/or want to be better educated on self-management techniques.
