Open to individuals in the service area who are disabled or over 65 who have lost SSI (and Medicaid) due to increase in Social Security. Applicants must meet income and resource guidelines and other non-financial requirements, call for more information.

Provides assistance to senior citizens and disabled individuals for long term care in a nursing facility, or services to help remain in the community rather than be cared for in a long term care facility. In-home services that are part of this program include home health aides, skilled nursing, medical day care and medical transportation.
