Description

Helps identify supports and individual needs, help the individual plan for the life they want, and works to connect the individual to services that will help a person achieve a quality life in the community and using a person-centered approach, to develop and maintain the Individualized Service Plan (ISP) which is necessary to ensure you, or your loved one, have the supports you need to live the life you choose.
