Individuals who are low-income, disabled, homeless, or veterans who are below 175% of the poverty level

Required documents

Referral form, proof of income eligibility, assistance including General Assistance (GA), Food Stamps/SNAP, and TANF, Social Security cards, a Certificate of Discharge (DD214) (if a veteran)
