Eligibility

Open to adults and adolescents within the service area who have a substance abuse addiction.

Required documents

Court documents, driver's license, DUI arrest report, income tax return, Medicaid card, photo ID, police report, prescription, proof of address, proof of age, Proof of benefits (i.e. EBT, TANF, GA), Social Security card/number, unemployment benefits statement, W-2 formCall for more information.

Fees

There is an intake fee (money orders only accepted) and a weekly service fee. Medicaid ABP and Plan A are accepted and a sliding scale fee system is available. The Fee for Service Initiative (FFS) and DUII are also available. Call for more information.

Description

Provides a range of outpatient services for individuals who have a substance abuse addiction. Services are coordinated by a case manager and may include individual, family, and group counseling sessions for adolescents and adults, social and recreational activities, educational and vocational services, and life skills training.
