Open to individuals who are HIV positive in the service area

Birth certificate, driver's license, financial statements, income tax return, insurance information, Medicaid card, Medicare card, photo ID, proof of address, Proof of benefits (i.e. EBT, TANF, GA), proof of disability

Description

Provides a variety of supportive services for individuals that are HIV positive. Programs include case management, chronic disease management, information and referrals, client advocacy, peer companions, referrals to dental care, community outreach and education and counseling.
