Connecting NJ | Southern New Jersey Perinatal Cooperative (SNJPC)

Assists pregnant individuals, parents, and caregivers of young children from diverse backgrounds to enhance their quality of life. Connects consumers to free or affordable wellness resources, identifies their needs, and links them to services such as health insurance, medical care, housing, childcare, prenatal health education, WIC applications, early childhood programs, home visiting programs, and community health workers.

Application process

Call to speak with an intake specialist. The specialist will assess the caller's needs, conduct a community health screening to gather information, and assist with determining what services will best meet their needs.

Eligibility

Open to pregnant women, families, parents and other caregivers of children ages birth to 5 years, including undocumented individuals in the service area.
