Projects for Assistance in Transition from Homelessness (PATH) | Cumberland County Guidance Center
Provides community-based case management and outreach services for adults diagnosed with a serious and persistent mental illness, and are homeless or are at risk of becoming homeless. Also provides targeted case management services for military veterans experiencing homelessness and mental health issues.
