Individuals, ages 18 and older, diagnosed with a serious mental illness and/or a substance abuse disorder, that are homeless or at imminent risk of becoming homeless.

Description

Provides community-based case management and outreach services for adults diagnosed with a serious and persistent mental illness, and are homeless or are at risk of becoming homeless. Also provides targeted case management services for military veterans experiencing homelessness and mental health issues.
