Program for Assertive Community Treatment (PACT) | Saint Clare's Health

Assist individuals with a history of repeated psychiatric hospitalizations, not benefiting from traditional community mental health services, by offering integrated rehabilitation, treatment, and support services. The program aims to reduce hospitalizations, enhance vocational opportunities, and improve the quality of life. It provides 24/7 availability of the PACT team, individualized and flexible services in home or community settings, medication prescription, administration, education, and support, assertive outreach, "in vivo" skills training, rapid crisis response, career counseling, job skills training, ongoing worker support, MICA treatment, direct assistance with critical life tasks, and assistance in developing peer/community social supports. PACT services are available lifelong if needed.

Eligibility

Open to adults, ages 18 or older, with a history of psychiatric hospital admissions for serious and persistent mental illness, impaired functioning who have been approved by the Division of Mental Health Services (DMHS). Include individuals with the following conditions: - Schizophrenia/other Psychotic Disorders - Major Depressive Disorders - Bipolar Disorders - Delusional Disorders - Schizoaffective Disorder
