Adult Family Care | Senior Citizens United Community Services (SCUCS)

Provides an affordable residential alternative to long-term care facilities for individuals who, due to disability or age, cannot live independently. AFC can place nursing home eligible residents in privately owned homes under the care of specially screened, trained and supervised care providers. Nurses and social workers are assigned to each participant with professional staff available 24 hours a day.

Application process

A physician approval is needed to access the program. In addition, an initial screening and RN assessment are also required.

Fee

The program offers services to both private pay participants and those eligible for Medicaid. To be eligible for Medicaid, a prospective participant must earn less than $2,022 per month and have assets less than $2,000. Those people with more than the above would be considered "private pay". The program cost for "private pay" is $2,260.25. The cost for a Medicaid client is partially paid through the Medicaid program.

Open to individuals, ages 60 or older, who: - Are nursing home eligible - Are oriented - Have bowel continence - Are ambulatory - Have no contagious diseases - Have no active psychosis
