Does not provide emergency housingOffers transitional housing, healthcare, and supportive services to up to 18 homeless men. Program stays typically last nine months to one year, with the option to extend up to two years if necessary. Case managers collaborate with residents and referring agencies to create Program Plans, addressing budgeting, substance use treatment, mental health counseling, transportation, employment, medical needs, and long-term housing options. If employment is unattainable for medical reasons, Case Managers assist in applying for financial support (social security, disability, HOPWA, etc.). Program Plans outline long and short-term goals, aiming for each client's self-sufficiency and permanent housing. Clients can access nursing services, attend group counseling, and meet weekly with their Case Manager to review progress and address new needs or issues. Family counseling is available as needed.
