Description

Offers a New Jersey Department of Health and Senior Services Medicaid Waiver program for older adults, ages 65 and older, and disabled individuals that offers care alternatives for those who would otherwise be at risk of placement in a nursing facility. The program is designed to provide supplement assistance that the individual receives from his/her natural support network, community agencies, and volunteer groups.
