Southern Ocean Medical Center | Diabetes Self-Management Education Classes (DSME)

Empowers individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training, guiding them to modify their behavior and successfully self-manage the disease. Facilitated by licensed health professionals, the course covers evidence-based topics such as the disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, prevention and treatment of acute and chronic complications, goal setting, problem solving, psychosocial adjustment, and preconception care and management during pregnancy if applicable. Classes can be conducted individually or in groups.

Application process

A physician's referral will be required before registering. Those without a referral can receive assistance in obtaining one. Patients must be current with their physician in order for assistance.Please call for more information and/or to schedule an appointment.

Fee

Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. Medicare covers 10 hours of training.Those not covered by insurance may be able to pay a reduced rate of $94.00 for nutrition counseling, which can include diabetes education.
