Eligibility

Youth within the service area aged 5-21 years who have mental, behavioral, or emotional health challenges, substance use challenges, and/or intellectual or developmental disability challenges.

Required documents

Must have a referral from contracted system administrator of NJ Children's System of Care (Perform Care), which Tri-County CMO can facilitate, call for more information.
