Program of Assertive Community Treatment (PACT) - Bergen County | Bridgeway Rehabilitation Services
Offered individuals with serious mental illness through the Program of Assertive Community Treatment (PACT). PACT believes in fostering successful community living by delivering flexible services. Goals encompass teaching self-management, promoting community integration, preventing hospitalizations, and enhancing quality of life. Services include 24/7 PACT team availability, personalized home-based support, medication management, crisis response, career counseling, and life skills training. PACT aims for positive outcomes like employment, education, supportive networks, improved family relations, and a substance-free life. Lifelong services are available as needed.

Fee
The program accepts Medicaid and offers a sliding scale fee system. However, no one is denied services due to an inability to pay.
Eligibility
Open to any Bergen County resident over 18 years of age with a history of psychiatric problems. Applicants must be diagnosed with one of the following psychiatric diagnoses: - Schizophrenia/other Psychotic Disorders - Major Depressive Disorders - Bipolar Disorders - Delusional Disorders - Schizoaffective DisorderApplicants must also meet one of the following criterion within the prior 18 months: - 2 or more State Hospitalizations - 1 State Hospitalization with 1 or more other psychiatric hospitalizations - 1 State Hospitalization with multiple screening center episodes - 2 or more STCF and/or County Hospital admissions - 1 STCF or County Hospital admission with 1 or more other psychiatric hospital admissions - 1 STCF or County Hospital admission with multiple screening center episodes - 2 or more involuntary psychiatric hospital admissions at private psychiatric hospitals
