Community Care Program for the Elderly and Disabled | Monmouth County Division of Social Services
Enables individuals to stay or return to their community instead of residing in a long-term care facility through a federal home and community-based program. It provides limited covered services such as home health, adult day care, transportation, case management, homemaker assistance, prescription aid, and respite care. Additionally, it includes the Caregiver Assistance Program (CAP) and the Assisted Living/Alternate Family Care program for those needing extra support. This initiative is now under Managed Long Term Services and Supports (MLTSS) for medical coverage.

Eligibility
Open to individuals eligible for nursing home level of care in Monmouth County. Applicants must be between the ages of 65 or older or between 18 and 64 and determined to be disabled by SSA or the Division of Medical Assistance and Health Services.
