Description

Provides case management services, linkages to mental health treatment, temporary/permanent housing (based on availability) and referrals to support services for homeless individuals with mental health problems. Individuals are referred to outside agencies for health services, substance use disorders, transportation, financial assistance, and rehabilitation programs.
