Capital Health - Pennington | Diabetes Self-Management Education Classes (DSME)

Empower individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training. Facilitated by licensed health professionals on an individual and/or group basis, the course guides participants in acquiring knowledge and skills to modify behavior and successfully self-manage the disease and its related conditions. Tailored to the needs, goals, and life experiences of each individual, the program adheres to evidence-based standards. DSME covers a spectrum of topics, including the diabetes disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, acute and chronic complication prevention and treatment, goal setting, problem-solving, psychosocial adjustment, and, if applicable, preconception care and management during pregnancy.

$400 for the entire course for self-pay clients. Most medical insurances, including Medicaid and Medicare, are accepted. Amount of coverage varies by insurance plan. Please call for more information.
