Eligibility
- 18 years of age or older - Diagnosis of a severe and persistent mental illness in accordance with DSM IV TR criteria - Have been placed on CEPP status coming out of the hospital - Demonstrate capacity to function independently - Demonstrate ability to perform basic ADL's. - Complete medical history - Satisfactorily complete a brief visit
