Capital Health - Hamilton | Diabetes Self-Management Education Classes (DSME)
Equip individuals with diabetes through collaborative Diabetes Self-Management Education Classes (DSME) and training, where they acquire the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions. Guided by evidence-based standards, the course, facilitated by licensed health professionals on an individual and/or group basis, delves into crucial topics including the disease process, treatment options, nutrition management, physical activity, medication utilization, blood glucose monitoring, acute and chronic complication prevention and treatment, goal setting, problem-solving, psychosocial adjustment, and preconception care for those with applicable needs.
