Application process

Call for information and/or screening. Screen for Community Services (SCS) is an automated tool that prescreens consumers for home and community based services. The automated system is built use the information given to automatically calculate a level of service needed. SCS can be conducted by telephone or in person.

Includes services such as: Notary Program: Notarizes simple documents for Salem County residents aged at least 60 or those under 60 with disabilities, with a photo ID requirement. Excludes Power of Attorney, Wills, and similar legal documents.
