Support Services | Family Transition Support Services
Provides services to at-risk youth and their families. Services include: intensive in-home counseling: individualized and family, Mentoring, a summer camp and an after school program for Mercer County residents, transportation for parents to the Youth Detention Center to visit children, Parent Empowerment Program (in-home parenting program), evaluation services (substance use, sexual misconduct, fire risk, needs assessments), and life skills development.

Eligibility
Open to youth who are referred by Perform Care/Children's System of Care (CSOC), DCP&P, Care Management Organization (CMO), or MR. The summer camp and afterschool program are for Mercer County residents only.
