Description

Provides comprehensive medical, behavioral health, and social services for older adults through a PACE (Program of All-Inclusive Care for the Elderly) model. The interdisciplinary team coordinates primary care, therapies, medications, transportation, adult day health services, meals, and home-based care to help participants remain safely living at home rather than in nursing facilities.
