Provides outreach, rapid response and homeless navigation services to vulnerable individuals in need and those experiencing homelessness. Offers services such as treatment, housing, employment, education, and legal assistance.The program also mobilizes Impact Teams to provide direct, rapid response outreach and immediate services. If funding is available, homeless individuals may receive help to return to their point of origin.
