A referral can be made via telephone or by coming into the office. An appointment is not necessary. If eligible for services, a caseworker will complete an intake and services will begin.

Projects for Assistance in Transition from Homelessness (PATH) offers community-based case management and outreach services for adults who are dealing with a serious and persistent mental illness and are homeless or are at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues.
