Description

Offers housing solutions for adults aged 21 and above with developmental and/or multiple physical disabilities through various options such as group homes, supported and supervised apartments, and independent living units. Each facility offers supportive services including case management, personal assistance, nursing services, rehabilitation technology access, training in independent living skills, transportation access, community integration activities, fully accessible residences, building maintenance, certified home health care, and rent subsidies.
