Application process

In order to register for this program, interested individuals should call the Middlesex County Office of Aging and Disabled Services for information. An intake form will need to be completed along with a liability release form.Updating the data on the card is the sole responsibility of the applicant. All data must be kept accurate and up-to-date.

Eligibility

This program serves Middlesex County residents, age 60 and older, and disabled residents with a medical condition and access to a computer.
