Description

Provides specialized case management services for adults who are registered with the New Jersey Division of Developmental Disabilities (DDD). Referrals are matched with a Jewish Family Service (JFS) Support Coordinator, who helps the client and his/her family create a Person-Centered Care Plan and then makes the appropriate referrals to connect the client to community resources, which address the client's needs.
