Open to adults who a major mental illness and reside in Hunterdon County. Applicants must be looking to live independently and have an understanding of their mental illness.

Description

Empowers individuals with major mental illnesses to live independently in the community through supportive services, assisting clients in accessing affordable permanent housing, linking to employment and educational programs, and working to decrease their hospitalization rate, while providing services such as housing search assistance, moving-in services, mental health/medication education, employment services, co-occurring disorder services as needed, supportive counseling, and 24/7 on-call support.
