Helps individuals, ages 18 and over, diagnosed with a severe mental health condition successfully integrate into the community, with a focus on achieving and sustaining independence for personal fulfilment. Services include: advocacy, linkage and referral, relationship building, daily living skills, and development of community-based supports.
